कमर दर्द, स्लिप-डिस्क और सियाटिका : कारण, बचाव और उपचार

कमर के निचले हिस्से में दर्द की समस्या से अक्सर हमें दो चार होना पड़ता है. ऐसा दर्द जो कमर के निचले हिस्से से शुरू होकर एक या दोनों टांगों में चलता हुआ महसूस हो, सियाटिका का दर्द हो सकता है. सियाटिका में प्रभावित टांग में झुनझुनी या सुन्नपन भी हो सकता है. इस रोग में रोगी गिद्ध के सामान लड़खड़ा कर चलता है इसलिए आयुर्वेद में इस रोग को ग्रध्रसी कहा गया है.

कमर दर्द: कारण


रीढ़ की हड्डी के निचले हिस्से से सियाटिक नर्व निकलती है जो दोनों ओर टांगों में जाती है.अधिकांशतः अनियमित जीवनशैली तथा उठने-बैठने के गलत तरीकों बढ़ती उम्र में इस सियाटिक नर्व और इसके आस पास के टिश्यू में सूजन आ जाती है जिसके कारण कमर के निचले हिस्से और टांगों में ज्‍यादा दर्द होता है खासकर कमर से लेकर पैर की नसों तक। साइटिका एक ऐसा ही दर्द है। दरअसल साइटिका खुद में बीमारी नहीं बल्कि बीमारियों के लक्षण हैं। इसका सूजन मूल कारण डिस्क प्रोलेप्स कमर के निचले हिस्से में  चोट या रीढ़ की हड्डी की आर्थराइटिस आदि हो सकता है.
आयुर्वेद शास्त्रों के अनुसार यह रोग एक वातव्याधि है. वात दोष के असंतुलन के कारण ही कटि-शूल या ग्राध्रासी होता है.  

कमर दर्द की चिकित्सा:


एलोपैथी में लक्षणों के आधार पर दवाइयां दी जाती हैं. दर्द के लिए पेन-किलर और नसों को ताकत देने के लिए विटामिनों की गोलियां, कैप्सूल आदि चिकित्सक देते हैं.
कई बार यदि डिस्क बढ़ने के कारण सियाटिक नर्व या उसके आस पास के टिश्यू पर दबाव ज्यादा होता है तो न्यूरोसर्जन सर्जरी के द्वारा बढे हुए हिस्सों को निकालने की सलाह देते हैं.
फिजियोथेरेपी में फ़िज़ियोथेरेपिस्ट की देख-रेख में लम्बर ट्रैक्शन और डायाथर्मी सिकाई से आराम मिलता है.

आयुर्वेद चिकित्सा:



आयुर्वेद में स्लिप डिस्क और सियाटिका के रोगी को चिकित्सक वातशामक व दर्द निवारक औषधियां जैसे गुग्गुलु, निर्गुन्डी, शल्लकी, रासना, दशमूल, कुपीलू, पिप्पली, शुंठी, मरिच, मेथी, अश्वगंधा, त्रिफला आदि एकल अथवा विभिन्न औषधि योगों के रूप में देते हैं, जो कि गोली, कैप्सूल, पुडिया या काढ़े के रूप में हो सकती हैं. एलोपैथी दर्द निवारक दवाइयों की तुलना में आयुर्वेदिक औषधियों को लम्बे समय तक लेने पर भी किडनी और लीवर पर दुष्प्रभाव सामान्यतयः नहीं होते हैं. इसके अतिरिक्त मालिश के लिए विभिन्न औषधि सिद्ध दर्द निवारक तैल जैसे; प्रसारिणी तैल, पंचगुण तैल, महाविषगर्भ तैल, बला तैल आदि रोगी की स्थिति के अनुसार मसाज के लिए देते हैं.
एक आयुर्वेदीय प्रक्रिया जिसे कटि-वस्ति कहते हैं, इस रोग में अत्यंत लाभकारी सिद्ध हुई है. इसमें आयुर्वेद चिकित्सक के निर्देशन में औषध-सिद्ध तैलों द्वारा कमर के निचले हिस्से की सिकाई की जाती है.

कमर दर्द से पीड़ित होने पर क्या करें व क्या ना करें?


·        सही स्थिति में कुर्सी पर बैठें
·        लम्बे समय तक एक ही स्थिति में ना बैठे, बीच बीच में टहलते रहे और पोजीशन बदलते रहें.
·        हल्का सुपाच्य संतुलित भोजन करें.
·        ऐसे आहार हो पचने में भारी होते हैं जैसे उड़द, छोले, राजमा, फ़ास्ट फ़ूड, मांसाहार आदि न लें.
·        आगे की ओर ना झुकें
·        अत्यधिक भारी वजन न उठायें.
·        ऊँची एड़ी की चप्पल न पहने
·        आराम करने के लिए तख़्त या सीधा बेड जिस पर हल्का गद्दा बिछा हो, प्रयोग करें
·        चिकित्सक के निर्देशानुसार यदि आवश्यक हो तो लंबर बेल्ट का प्रयोग करें.
·        व्यायाम या योगासन किसी कुशल व्यक्ति के निर्देशन में ही करें.

AIDS and Possible Ayurvedic Management

Introduction of AIDS:
  • AIDS is the end stage of a progressive state of immune deficiency.
  • Causative Organism: Human Immunodeficiency Virus (HIV)
  • HIV was first discovered in 1983 by Barre-Sinoussi and Montagnier
  • The first case of AIDS was detected in UK
  • Confirmed HIV infection with CD4 T lymphocyte count is < 200000/L with symptoms
  • AIDS is common in Africa and Asian countries
  • HIV mainly  harbours in semen, genital secretions, blood, pus, sputum, saliva and other body fluids
Clinical Classification of HIV infection:
  • Acute infection
  • Asymptomatic but positive HIV
  • Persistent generalised lymphadenopathy
  • AIDS (HIV related disease):
    Constitutional diseases like weight loss, fever, diarrhoea;Neurological diseases, dementia, neuropathy, myelopathy;Opportunistic infections;Malignancies i.e. Kaposi’s sarcoma, non-Hodgkin’s lymphomas, primary cerebral lymphomas;Other diseases attributable to HIV infection.
Mode of Transmission:
  • Sexual intercourse- Vaginal or Anal
  • Needle pricks- Using unsterilized needles for injections, in IV drug abusers, careless handling
  • Mother to child- During  birth through vaginal secretion, trans-placental, through breast milk
  • Blood transfusions
  • Organ transplantations
Investigations for HIV:
  • ELISA test (screening test)
  • Western blot test (diagnostic test)
  • Polymerase Chain Reaction (PCR)
  • Anti-HIV antibody detection
  • Viraemia quantification- To start treatment and to see the response of antiviral drugs
  • CD4 T lymphocyte Count
Window period:After HIV infection, there is a time gap for the patient to become reactive to tests. This time gap is called as ‘Window period’. This period is variable. But during this period,  the individual is infective.






General Symptoms in HIV:
  • Weight loss more than 10%
  • Fever more than 1 month
  • Diarrhoea more than 1 month
  • Neuralgia, arthralgia, headache
  • Lymphadenopathy
  • Cutaneous rashed, dermatitis, fungal/ bacterial/ viral(herpes simplex 1 and 2) infection
  • Dental infection, gingivitis, candidiasis of oral cavity and oesophagus
  • Varicella zoster infection
  • Opportunistic infections
  • Poor healing after surgery, trauma, infection with more complications
Tumours in HIV infection:
  • Kaposi’s sarcoma- 40% common
  • Lymphomas- 3 to 4 %
  • Cervical cancer
  • CNS lymphomas
  • Ano- genital squamous cell carcinoma
  • Testicular tumours
  • Lung cancer
  • GIT lymphomas and adenocarcinomas
  • Squamous cell carcinoma of anal canal and cervix
Respiratory problems in HIV infection:
  • Pneumonia
  • Tuberculosis
  • Fungal infections
Gastro intestinal tract problems in HIV infection:
  • GIT infections- bacterial, protozoal, viral
  • Kaposis’s sarcoma, lymphomas, adenocarcinomas
  • Hepatitis (‘C’ virus), cholestasis
  • Anorectal disease
  • Abdominal tuberculosis
Neurological problems in HIV infection:
  • Encephalitis, aseptic meningitis, myelitis
  • Neuropathies with demyelination
  • Opportunistic infections like Toxoplasma, Cryptococcus causing severe meningitis

Management in AIDS patients:Investigations:
  • Tests for HIV

Prophylactic measures to be adopted by surgeons while treating AIDS Patients(Universal Precautions):
  • Proper care should be taken to prevent transmission of the virus
  • Care in handling sharp objects like needles, blades
  • Disposal of needles through a sharp disposing container
  • All cuts and abrasions in an HIV patient should be covered with a waterproof dressing
  • Equipments and areas which are contaminated with secretions should be wiped with 2% glutaraldehyde solution
  • Equipments should be disinfected with 2% glutaraldehyde solution
  • Contaminated gloves, cotton should be burned
  • Disposable equipments (drapes, scalpels etc.) should be used whenever possible
  • Walls and floor should be cleaned properly with soap water
  • Separate operation theatre and staff to do surgeries to HIV patients is justifiable
  • Avoid shaving whenever possible before surgery in HIV patients
  • All people inside the theatre should wear disposable gowns, plastic aprons, goggles, overshoes and gloves
  • Surgeons, assistants and scrub nurse should wear in addition double gloves
  • Suction bottle should be half filled with freshly prepared 2% glutaraldehyde solution
  • Spilled body fluids should be diluted with 2% glutaraldehyde solution
  • Accidental puncture area in surgeon or scrub nurse should be immediately washed with soap and water thoroughly
  • Operation theatre should be fumigated after surgery to HIV patient
Possible Ayurvedic treatment for AIDS patients:
  • As we all know that AIDS is one of the contagious and sexually transmitted diseases, so proper care should be taken to prevent transmission of the virus.
  • As per Sushruta Samhita (School of Ayurvedic Surgery),
    प्रसंगात् गात्रसंस्पर्शात् नि:श्वासात् सहभोजनात् |सहशैयासनाश्चापि वस्त्रमाल्यानुलेपनात् ||कुष्ठं ज्वरश्च शोषश्च नेत्राभिस्यंद एव च |औपसर्गिकरोगाश्च संक्रामन्ति नरात् नरम् ||Means, if a person adopts following behaviour with patient having contagious diseases, then it leads to manifestation of that same disease in that individual.
  • Constant close intimacy with the patient
  • Often touching the patient
  • Inhaling the exhale air of the AIDS patient
  • Eating together repeatedly
  • Sleeping and sitting together repeatedly
  • Wearing dress, garlands and unguents use by patient who is suffering from contagious disease like skin disease, viral fever, tuberculosis etc.
  • Diseases such as AIDS are difficult to treat because of the nature of the diseases, and also particularly when it is associated with the complications or secondary diseases. As per Sushruta Samhita(Ref: Su. Su.33/3), it can be curable by some extent with the use of ‘Rasayana Dravyas’ (Ayurvedic Rejuvenation Drugs/ Ayurvedic Anti Ageing Drugs).
  • When AIDS patient is suffering from fever, diarrhoea or weight loss, he should have to consume boiled water mixed with dry ginger and coriander seeds powder every time, which will help to improve the digestive power as well reduced the inflammation of intestinal wall. It also restores the fluid loss and saves the patient from dehydration.
  • AIDS patient should never suppress the natural physical urges as hunger, thrust, sleep, sneezing, yawning, vomiting, defecation (bowel evacuation), urine, flatus and semen ejaculation.
  • When AIDS patient is affected with respiratory problems, he should have to take ‘Sunthyadi Yoga’(Contains of Dry ginger+ Piper longum+ Pomegranate+ Gooseberry+ Barley+ Kulathth+ Clarified butter (Ghee) and Goat meat juice). This remedy will completely eradicate the respiratory problems of the AIDS patient.
  • As per Ayurveda, room of the AIDS patient should be daily fumigate with the Ayurvedic herbal mixture of Guggulu, Aguru, Sarjaras, Sarsap, Vacha, Nimb patra and  Ghee (Clarified butter). (Ref: Sushrut Samhita Su. 5/11- Rakshogna Vidhan)
  • AIDS patient should practise Ayurvedic oil massage regularly with ‘Mahanarayan Oil’. It makes the skin supple; it controls Vata Dosh(body humor) by the opposite properties; it enhances peripheral blood circulation and encourages quicker removal of metabolic waste; relaxing the body. He/she become resistant to exhaustions and exertions; the body, even if subjected to injuries or heavy and stressful work is not much injured. One’s physique becomes smooth, flabby, strong and charming. By applying the oil massage regularly the onslaught of aging is slackened. Follow the normal direction of hair growth; use a little extra oil over the body’s vital parts. Do massage whole over the body, but in case of short of time, at least do massage of Ayurvedic oils on scalp/head, ears & soles of both feet daily.
  • After having Ayurvedic oil massage, he/she should go for ‘Udavartana’ (massage by Ayurvedic herbal powder mixture of Jivanti, Durva, Manjishtha, Ashwagandha, Apamarg, Agnimanth, Yashtimadhu, Bala mool, Sarshap, Atasi fal, Kushtha, Rice, Mash, Til, Barley flour and the liquids of wine, curd and Honey) whole over the body, through which the body eliminates bad smell, excessive sweating, and feeling of heaviness in the body, drowsiness, itching and even removes undesirable dirt.
  • After that, he/she should go for Bathing. It is purifying; improves vigour, vitality and life-span; removes fatigue, sweating and dirt of the body. It brings about strength in the body and is an excellence for the enhancement of Ojas (lustre).
  • One should use of perfumes or aftershaves which stimulate vigour and vitality, produces good smell in the body and enhances longevity and charm; it gives corpulence and strength to the body; it is pleasing to the mind and it prevents inauspiciousness.
  • Wearing of gems and ornaments adds to the prosperity, auspiciousness, longevity, grace; prevents dangers from snakes, evil spirits, etc. It is pleasant and charming. It is also enhancing the Ojas (lustre).
  • For become relax, one should go for a walk, spend time in nature, make company with good friends, listen to music, watch  a comedy, play  with a pet or work in your garden.
  • All events in this universe are under the control of scientific circumstantial evidence. There is no independent doer. Lack of this knowledge creates worries. So attend the self realisation programme which will make you free from all the worldly attachments and worries.

गर्मी के मौसम में त्वचा को रखें स्वस्थ

गर्मी के मौसम में त्वचा को रखें स्वस्थ
गर्मी के मौसम में करेंगे ये छोटे-छोटे काम तो चेहरा चमकने लगेगा
पेट साफ रहे तो त्वचा की समस्याएं नहीं होती हैं। हाजमा ठीक न हो तो चेहरे पर कील-मुंहासे हो सकते हैं। इसीलिए स्किन को स्वस्थ रखने के लिए कब्ज को दूर करना बहुत जरूरी है। पेट साफ करने के लिए प्रतिदिन सुबह-सुबह गुनगुने पानी में शहद की कुछ बूंदें डालकर पिएं। इससे कील-मुंहासे की समस्या समाप्त हो जाएगी।
दो छोटे चम्मच बेसन में आधा छोटा चम्मच हल्दी मिलाकर खूब फेंटें। फिर इस लेप में दस बूंद गुलाब जल व दस बूंद नींबू मिलाएं। इस लेप को लगाकर चेहरा धो लेने से स्किन ग्लो करने लगती है और चेहरे पर मौसम का प्रभाव नहीं दिखाई पड़ता है।
पानी ज्यादा पिएं। रोजाना कम से कम दस गिलास पानी पिएं, क्योंकि पर्याप्त मात्रा में पानी पीने से शरीर के विषैले पदार्थ बाहर निकल जाते हैं और त्वचा स्वस्थ और चमकदार बनती है। अधिक पानी पीने से त्वचा पर झुर्रियां नहीं पड़ती हैं।
बार-बार ठंडे पानी से चेहरा धोते रहें। साबुन या फेसवॉश का प्रयोग कम से कम ही करें। अपना टॉवेल अलग रखें।चेहरे की गंदगी और धूल-मिट्टी साफ करने के लिए टमाटर के टुकड़े चेहरे पर धीरे-धीरे रगड़ें। यदि आपके चेहरे पर दाग-धब्बे पड़ गए हैं तो नींबू के छिलके पर थोड़ी-सी चीनी डालें और फिर छिलके को दाग-धब्बे वाले स्थान पर धीरे-धीरे रगड़ें।
स्ट्रॉबेरी में विटामिन सी और एंटीऑक्सीडेंट भरपूर मात्रा में पाए जाते हैं। स्ट्राबेरी को अपने चेहरे पर मैश करके लगाएं, चेहरा ग्लो करने लगेगा
केले में बहुत अधिक मात्रा में माइश्चचराइजर पाया जाता है।केले में शहद मिलाकर चेहरे पर लगाएं और 10 मिनट बाद धो लें। यह चेहरे पर ताजगी लाता है।
संतरे के छिलके को सुखा लें और पीसकर पाउडर बनाएं। इस पाउडर में थोड़ा-सा कच्चा दूध मिलाकर चेहरे पर लगाएं। सूखने के बाद ठंडे पानी से धो लें, स्किन स्मूथ हो जाएगी।
पके पपीता के गूदे को चेहरे पर मलें। सूखने पर धो लें, चेहरा चमकने लगेगा।
आपकी स्किन बहुत ज्यादा ड्राय हो तो एक छोटा चम्मच मलाई लेकर उसमें थोड़ा-सा केसर मिलाएं। मलाई से चेहरे की मालिश करें। सूखने पर चेहरा धो लें। रूखापन दूर हो जाएगा।
एक चम्मच शहद में एक चम्मच पानी मिलाकर इस पेस्ट को चेहरे पर लगाएं। कुछ देर बाद चेहरा धो लें, स्किन ग्लो करने लगेगी।
काजू स्किन के लिए बहुत अच्छा होता है। काजू को दूध में भिगोकर उसे पीस लें और चेहरे पर लगाएं। स्किन ड्राय हो तो काजू को रात भर दूध में भिगो दें और सुबह पीसकर इसमें मुल्तानी मिट्टी और शहद की कुछ बूंदें मिलाकर स्क्रब करें।
आधा चम्मच संतरे का रस लेकर उसमें 4-5 बूंद नींबू का रस, आधा चम्मच मुल्तानी मिट्टी, आधा चम्मच चंदन पाउडर और कुछ बूंदें गुलाब जल की मिलाकर थोड़ी देर के लिए फ्रिज में रख दें। इसे लगा कर 15-20 मिनट तक रखें। फिर पानी से चेहरा धो लें।
मुल्तानी मिट्टी में दही और पुदीने की पत्तियों का पाउडर मिलाकर लगाएं। आधे घंटे तक रहने दें। सूखने पर गुनगुने पानी से चेहरा साफ कर लें। ये नुस्खा तैलीय त्वचा को हेल्दी बनाता ह

Sinusitis is an inflammatory condition of the sinuses. Sinuses are air filled spaces in the skull. There are 4 pairs of sinuses. These give a structure to the face and also makes the skull weigh less. They also serve many other important functions like protecting the internal delicate structures of the skull by acting as a shock absorber etc. These air spaces are lined by a layer of mucus membrane. All the sinuses open into the nasal cavity. These sinuses ensure free air circulation and conditioning of the air so that the environment inside the skull is maintained in a state of equilibrium.
The problem arises when this mucus layer gets inflammed and produces more mucoid substance and they get occluded inside the air space. This results in the typical symptoms of this disease like heaviness of head, head ache, running nose etc. Inflammation of the mucous membrane occurs due to some viral or bacterial infections. This condition can also occur when the opening of the sinus is occluded by a deviated nasal septum or polyp and there is improper drainage of the mucous from the sinus.

Symptoms:
The pain in sinusitis depends on the sinus which is affected. Pain and tenderness over the eye brows and mid forehead indicates infection of frontal sinus. Pain over the cheek bone indicates infection of the maxillary sinus. Other symptoms include,
  • Fever
  • Tiredness
  • Headache
  • Nasal congestion and running nose sometimes
  • Swelling of the face
  • Post nasal drip
  • Cough
  • Sore throat
Management Of Sinusitis:
Viral sinusitis requires not treatment and gets resolved on its own in 2 weeks. When there is a secondary bacterial infection, this condition is treated with antibiotics, steroids and other symptomatic treatment for fever, pain etc is administered. In cases of chronic, unresolved and recurrent attacks of sinusitis, surgical procedures like septoplasty, FESS etc is resorted to.
In Ayurveda, this condition can be corelated to Dushta Pratishyaya, where the main dosha affected is Kapha. It get aggravated and vitiates the Prana Vata (a sub type of Vata), which is mainly present in the respiratory tract. In this condition, the kapha is affected by Aama, the undigested sticky remains of food which is a resultant of improper Digestive fire. Hence a correct line of treatment will be first
  • Rectifying the digestive fire,
  • Clearing the Aama,
  • Pacifying the vitiated Kapha and finally
  • Ensuring proper movement of the Prana Vata through the respiratory passages.
For bringing out the Aama and polluted kapha from the body, Panchakarma is the best tool. Especially, in this condition, Shodhana Nasya has to be used. With this, all the blocked up mucous in the sinuses which are a form of the Kapha comes out which gives great relief to the patients. This treatment is followed by inhalation of fumes which clears the residual kapha from the passages. Once the passages are cleared, Vata can move easily in its path. This is indicated for chronic and recurrent cases of sinusitis. Acute cases can be effectively managed with internal medicines, steam inhalations and hot fomentation over the painful parts of the face and mild massage. This gives the desired effect of liquifaction and drainage of kapha. Internal medicines for improving the digestion and regulation of bowel is an important aspect of management.
People having chronic and recurrent sinusitis will be greatly benefitted by Yoga and Pranayama. The breathing practices involved in Pranayama helps in proper circulation of Prana Vata which effectively drains the nasal sinuses and prevents any blockages from developing. Jala neti kriya is very effective for this condition.

DIET AND LIFESTYLE:
Since kapha is the main causative factor for this condition, one should avoid food and activities which causes its disturbance.
  • One must avoid cold, oily and heavy foods.
  • Digestion and bowels should be regulated with healthy and light foods.
  • Head bath with cold water must be avoided.
  • After head bath, one can apply Rasnadi churnam on the vertex of the head which will prevent seepage of moisture into the sinuses.
  • Ice creams and cold beverages should be strictly avoided.
  • At the first sign of flu and common cold, one must take precautionary measures like steam inhalation, fumigation, hot fomentations and warm soups etc.
  • Regular use of Nasya with Anu tailam is beneficial.
  • Regular practice of yoga and Pranayama.
  • Day sleep should be avoided.
  • Avoid exposure to cold.
  • Use of warm caps and coats while going out in the cold. 
Alternative treatments like Ayurveda, Reiki and Hypnosis helps in treating Sinusitis.

Premature Ejaculation means;
Premature Ejaculation should be termed as persistently or recurrently early orgasmic response. It indicates a state of gap between the person’s expectation and his performance vis-à-vis his orgasm.

  • Squeeze technique and stop-start technique:
    Both the techniques are based chiefly on understanding sensation premonitory to orgasm. Participation and cooperation of the partner, tranquil condition and improving of relationship are very much essential for this treatment. The treatment consists of manual stimulation of penis by the partner until he reaches a degree of excitement approaching impending orgasm. Just prior to the stage of orgasmic or so called ejaculatory inevitability, the male signals the partner to stop further stimulation or at this point the partner applies steady pressure squeezing the glans penis by thumb at frenulum and index and middle finger on either side of the coronal ridge (on glans and the shaft respectively) for 3 to 4 seconds or until erection decreases. The man should relax, be in touch with his own sensations and not get worried during this technique about partner’s sexual needs. The squeezing process needs to be resumed three or four times before orgasm occurs. After three or four successful sessions he may proceed to vaginal containment in female above position and later side to side and male above position, depending upon the individual progress. A few weeks treatment often cures this condition.

Sitz bath; a wonder remedy in anorectal problems
Sitz Bath:
The name is derived from the German verb ‘sitzen’ meaning ‘to sit.’
Definition:
A sitz bath (also called a hip bath) is a type of bath in which only the hips and buttocks are soaked in water or saline solution.
In Ayurveda it has been described as ‘Kati Snaana’
Equipment used for sitz baths:
Sitz bath apparatus
A sitz bath, in which only the hips and buttocks are soaked in water or saline solution, is used for patients who have had surgery in the rectal area or to ease discomfort from fissure, painful hemorrhoids, bladder, prostate, or vaginal infections etc.
Purpose:
A sitz bath is used for patients who either have had surgery in the area of the rectum, or to ease the pain of Anal fissure or an abscess in anal or perianal region or thrombosed/strangulated external hemorrhoids, uterine cramps, prostate infections, painful ovaries, and/or testicles. It is also used to ease discomfort from infections of the bladder, prostate, or vagina. Inflammatory bowel diseases are also treated with sitz baths.
Preparation
The bath should be filled with 3-4 in (8-10cm) of water. For most conditions, nothing else should be added (no bubble bath or oil).
Description
Warm sitz baths are one of the easiest and most effective ways to ease the pain of painful anorectal conditions like Fissure in ano, Thrombosed/strangulated haemorrhoids or an abscess in the anorectal region.
A brief, cool sitz bath helps ease inflammation, constipation, and vaginal discharge. It can be used to tone the muscles in cases of bladder or bowel incontinence.
How to take a sitz bath?
  1. A sitz bath tub can be easily purchased at any general/plastic utensils’ store. Select the size that suits you. Sometimes you may also find them at hospital pharmacies or medical supply stores.
  2. Prepare the items you will need so they are near at hand from where you will be using the sitz bath.
  3. Place the sitz bath tub over the horizontal surface of bathroom, and fix it at its place.
  4. Fill the sitz bath with warm water. Remember that the water temperature will feel cooler to your hands than it will to the rest of the body.
  5. If your doctor has recommended it, add salt or alum powder to the water.
  6. Sit in the sitz bath for 10-20 minutes, or as recommended by your doctor.
  7. When finished, use a towel to gently pat the affected area dry.
  8. Use your sitz bath as often as recommended by your doctor; generally several times a day as needed to ease discomfort.
  9. Wash your sitz bath tub after each use and dry it thoroughly.
Precautions
Some patients may become dizzy when standing up after sitting in hot water; it is best to have someone else present when doing a sitz bath.
Aftercare
The area should be carefully patted dry and, if necessary, clean dressings should be applied.
Risks
Sitz baths pose almost no risk. On rare occasions, patients can feel dizzy or experience rapid heart beat because of blood vessel dilation.
Normal results
Swelling goes down; discomfort is eased; healing is promoted.

Fistula-in-ano and Ayurveda Kshar-sutra therapy


The history of medical literature available today very clearly speaks that the disease Fistula-in-Ano (Bhagandara) affects more reputations of surgeons who deals with it. There is a saying in medical world "the best way to take revenge of a surgeon is to refer him a patient of Fistula-in-Ano". Thus, it is a well known fact that in spite of the tremendous developments of modern medical science especially of surgery, the disease Fistula-in-Ano still remains a challenging problem to the medical world.
The ano-rectal disorders and their management has established a separate surgico-dynamic  speciality among the various surgical disorders. The ano-rectal disorders have been known form the very early period of evolution to the medical science. The disease Bhagandara (Fistula-in-Ano) is said to have existed about four thousand years according to Wilson (1963) as mentioned by Khurana et al (1972). Furstenbergh et al (1964) have mentioned the method of Apolinose by which Hippocratus, the Father of modern medicine (460 BC) treated Fistula-in-Ano.
The main limitation of treatment of Fistula-in-Ano by conventional surgery is that even after surgical excision of the tract the recurrence rate still remains between 20-30%. Hence this disease even today stands as a challenge in front of the medical world.
Susruta’s method of thread treatment is known as Kshara-sutra treatment as mentioned above and even today it is quite successful method in treating this difficult disease.  Hippocrates named the procedure of passing the ligature along the fistulous tract as Appolinose. Chandsi and Madrasi treatments are known by their names only and much detail are not known about them. However it was Prof. P. J. Deshpandey at Banaras Hindu University, who took the lead after ancient Acharyas for exploring the technique, and modified and standardized it with the help of modern science and technology. The treatment was put to clinical trials in patient of Bhagandara and many scientific papers published. Apart from that, at the Department of Shalya Shalakya, Gujrat Ayurved University, Jamnagar many works done on Kshara-sutra treatment for the disease Bhagandara under the guidance of Prof. Kulwant Singh. Later on various other places in India and abroad started Kshara-sutra treatment in Bhagandara (Fistula-in-Ano) and piles patients. The Kshara-sutra treatment is now a well accepted Ayurvedic technique for treating the ano-rectal diseases viz. Fistula-in-Ano, Arshas (Piles), Parikartika (Fissures with tags) etc.

APPLICATION OF KSHARA-SUTRA IN A PATIENT OF BHAGANDARA (FISTULA-IN-ANO):
Pre Kshara sutra (Pre-operative):
a.      The patient should be admitted in the hospital a day before operation.
b.      Laxative like Panchsakara churana is given at previous night
c.       Shave and part preparation done.
d.      Patient is kept fasting for at least 8 hours
e.      Consent of the patient in written
f.        Proctoclysis enema 2-4 hours before the procedure
g.      Inj. Xylocaine sensitivity test
h.      Inj. Tetenus toxoid 1 Amp. IM stat.

EQUIPMENTS AND OTHER REQUIREMENTS

Kshara sutra

POSITION: All the patients were given lithotomy position for the  procedure.

Kshara sutra threading procedure (Operative procedure):

  1. Spinal anesthesia is induced by the Anesthetist.
  2. The patient is made to lie on the operation table in Lithotomy position.
  3. The perianal area is cleaned with Savlon and spirit followed by Betadine painting.
  4. The outer area is covered with sterile cloth, leaving the anal area open.
  5. Digital examination done to locate any induration, internal opening and to exclude other lesions.
  6. Probing is done through a malleable probe to locate the internal opening with the index finger of other hand inside the anus. If internal opening is located, the probe is pushed out through the anal verge and the track is threaded loosely by Kshara sutra. However if internal opening couldn’t be located it is made inside the anal canal at a point which offer least resistance to the tip of the probe. Again Kshara sutra threading of the track is done loosely.
  7.  10 ml. of Jatyadi oil is pushed inside the rectum and sterilized gauze is applied on the anus covering the external opening.
  8.  T-bandage is tied to keep the dressing in proper position.
  9.  Thereafter the patient is shifted to the ward.

Post Kshara sutra threading management (Post operative):
1.       Nil orally for 4 hours
2.       Give liquid diet after that to avoid any type of inconvenience.
3.       Monitor the vitals
4. If operation is done under spinal anaesthesia, to avoid   complication give head-low position for 12 hours.
5.       Jatyadi oil P/R OD
6.       Hot sitz bath with panchvalkal kwath 8 hourly
7.       To avoid hard stool, give mild laxative to the patient
8.       If pain is excessive, urinary retention occurred, it is managed accordingly.


Post Kshara sutra threading complications:

1.   Retention of urine – it has been observed that within 8 to 10 hours after ligation, some of the patients complain of retention of urine which can be tackled by frequent sitz bath in lukewarm panchvalkala kwatha or simple warm water. Catheterization is seldom required.
2.  Local irritation – In some of the patients local i.e. perianal irritation is seen which needs frequent use of oil application and hot sitz bath etc.
3. Abscess formation – In some of the patients (especially who was suffered from interno-external piles), abscess formation takes place which was managed with local application of Dashang lepa with Goghrita.
4.  Haemorrhage – Alarming type of haemorrhage is not a rule with Kshara sutra treatment. However in some of the cases slight oozing may be seen which need no special care except the usual routine line of management, viz. avoidance of hard stool and much straining during defecation.

Weekly change of the sutra: - Kshara-sutra applied initially is kept for 7 days after which it is replaced by a fresh Kshara-sutra by Rail Road Technique. This procedure is repeated every week till the track completely cut through and if there is any unhealthy granulous tissue, it is scrapped-off during this weekly follow-up to promote a better healing.

Mode of action of Kshara sutra:
Kshara sutra works by pressure necrosis, chemical cauterization by kshar (alkali) and sloughing of the tissue of the walls of the fistulous track along with adequate drainage. It leads to an easy debridement of unhealthy tissue and pus etc. and thus providing a cleaner base for the wound healing of the fistulous track. The Kshara-sutra is changed weekly so that an average pace of cutting of about 0.5-0.8 cm/week is maintained along with healing from behind. Finally the whole track is cut through and the fistula gets healed up with minimal scarring and without any other major complication.

Pathya-apathya (Diet and routinely activities):
From the very first day of Kshara sutra threading procedure, light diet like Khichri is advised. Patient is also advised to take plenty of fluids, blend diet, green vegetables, salad and seasonal fruits. Patient is further advised to avoid spicy and fried food and not to strain during defecation.

Picture 2: A completely healed case of Fistula-in-ano after kshar sutra treatment. In this case there was done fistulectomy which was not successful and fistula recurred. Thereafter the threading of the track was done by kshar sutra and the fistula healed successfully in about 4 months without any recurrence. 

NOTE: The procedure can also be performed under local anaesthesia or without anaesthesia at all if the patient is co-operative. However  if the patient is sensitive (Vata dominant prakriti), spinal anaesthesia is the best choice.
